This release and waiver of liability filled out by the signer releases Solana Center for Environmental Innovation from any legal responsibility related to the activities performed by the individual while in service. Said individual (or guardian) acknowledges that they are choosing to participate and understand that they hereby release and forever hold harmless Solana Center, its Board of Directors, employees, volunteers and contributors from any and all liability, claims, and demands, either in law or in equity, which arise now or hereafter based on the activities performed.
This form releases Solana Center from any liability or claim that the individual (or guardian) may have with respect to bodily injury, illness, or death while performing activities as a participant. The individual (or guardian) understands that Solana Center does not assume any responsibility or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness. The individual (or guardian) acknowledges that Solana Center does not carry or maintain health, medical, or disability insurance coverage for participants and/or volunteers and each participant and/or volunteer is expected and encouraged to obtain his or her own health insurance coverage.
By checking this box, the participant (or guardian if under 18 years of age) grants the Solana Center for Environmental Innovation permission to use photographs of the individual or individual’s child/children in any and all of its publications, including website entries and social media venues, without payment or any other consideration. The Solana Center can edit, copy, exhibit, publish or distribute the photos without prior approval. Participant waives the right to inspect or approve the finished product, and will receive no financial compensation and have no rights of ownership.
